Community Health Worker

Community Health Workers connect populations to health services, deliver culturally-grounded health education, provide navigation and care coordination, and address the social determinants shaping community health outcomes.

The Five Components Behind Every Placement

Every Scholar moves through the same five-component service model, and every partner organization plugs into the same infrastructure. We recruit community-embedded candidates, place them into paid fieldwork, match them to certified job placements, keep them connected through the alumni network and career upskilling, and track outcomes across every step. Together, these five components turn credentials into durable careers and give partner organizations the reliability they need to plan around this workforce.

Recruitment and workforce production

Community-embedded participants ages 18 through 24 are recruited from the communities the host college serves and enrolled through the college’s existing CHW pathway. In partnership with the host college, HUMANDREAM works to enhance a placement partnership ecosystem that reflects the demographic and cultural composition of the region each college anchors

Paid fieldwork placement

HUMANDREAM builds and maintains executed Memoranda of Understanding with Federally Qualified Health Centers, school-based health centers, community mental health organizations, Enhanced Care Management providers, and Community Supports providers. Scholars complete their fieldwork hours at these sites, gaining supervised clinical exposure and real-world experience with the populations and settings that will hire them post-graduation.

Alumni network and career continuity

An alumni network for placement support, continuing education, and career progression, providing host college graduates with a longer-horizon connection to the health workforce than a standalone certificate typically offers. This includes visibility into transfer pathways and downstream Community Wellness Coach and Behavioral Health workforce roles.
